General Statement of Job
The Patient Navigator (PN) facilitates navigation of CL Brumback Primary Care Clinics patient’s comprehensive care. PN provides trustworthy, impartial medical information to help patients make sound decisions about their health and wellness. PN understand patient conditions and facilitates meaningful provider communication. PN carries out outreach and education to encourage patient’s engagement and facilitates the specific program and strategies to be implemented. PN works closely with the healthcare team at the Clinics including providers, nurses, practice managers, mental health professionals, medical director etc.
Specific Duties and Responsibilities
Implements the Chronic Disease Management Model for patients diagnosed with hypertension, diabetes, depression, and other chronic diseases.
Emphasizes the central role of patients and their relationship with clinic team to achieve optimal health outcomes.
Assists individual patients to improve self management skills and evaluate the overall effectiveness of chronic care management in the clinic setting.
Supports patient self-management skills through health coaching techniques.
Facilitates and follows plan of care with individual patients regarding his/her chronic conditions. PN uses tools such as “My Action Plan” and helps patient set a goal for behavior that they wish to change.
Engages patient in a discussion of an action plan so they can fulfill their goal.
Works with patients to develop behavioral-change plans usually focused on compliance with medication and treatment plans, following up on self-monitoring tools (such as BP and/or AccuCheck logs etc.),

High school diploma or GED required. Bachelor’s Degree in Business or related field preferred, preferably healthcare.
Three (3) to five (5) years in a healthcare setting preferably in the CMA role
Valid Florida Driver’s License required.
