The Outreach Case Manager is a member of the Services and Housing to Access Recovery Program (SHARP). Persons referred through Coordinated Entry for All are the priority for this grant, specifically Single Adults in King County experiencing Chronic Homelessness and Co-Occurring Mental Health and Substance Use disorders. This position is responsible for providing outreach, engagement and intensive case management services. This position provides on-going case management in streets, shelters, and consumer homes.
MAJOR DUTIES AND RESPONSIBILITIES
- Provide outreach and engagement services, as assigned, on the streets, in shelters and, on occasion, in outlying King County areas. Efforts are balanced between initial outreach and follow-up activity.
- Perform assessments (as trained) for accessing housing & services, conduct intake evaluations and mental health assessments; with client participation, develop support and treatment plans; facilitate linkages to long-term treatment resources when appropriate.
- Assume primary responsibility for coordinating all aspects of clients' support and treatment plans. This includes providing and/or coordinating the following: survival services with a focus on access to food, shelter and clothing; fastest possible acquisition and stabilization of housing; acute and ongoing medical care, psychiatric treatment, financial assistance, criminal justice system coordination, and substance abuse treatment.
- Advocate for clients' access to community resources and services, ensuring that clients' needs are met and rights maintained; consult and collaborate with community providers to ensure continuity of care.
- Participate in education of service community regarding the needs of homeless clients and present with co-occurring disorders, techniques of engagement and service provision.
- Participate in psychiatric consultation, supervision, program meetings and in-service trainings; participate in clinical reviews and case conferences for clients on caseload.
- Current Washington State, Department of Health minimum credential as an Agency Affiliated Counselor in good standing.
- Experience working with adults who are experiencing or who have experienced homelessness, have a mental illness and/or co-occurring substance abuse disorder.
- Valid Washington State Driver's License, reliable vehicle and insurable driving record.
- Interest or experience in working with participants who are difficult to engage and refer to traditional programs.
- Ability to communicate and work effectively with staff from various backgrounds.
- Ability to communicate effectively with clients displaying a wide range of unpleasant and/or bizarre behavior.
- Subscribe to the philosophy of cooperation and continuity across programs and of consideration and respect for clients.
EDUCATION AND EXPERIENCE
- Relevant Bachelor's degree in social work, psychology, or related behavioral science, or
- Bachelor's degree in any field and 1 year of paid work experience in social services, or
- Relevant associates degree and 1 year of relevant paid work experience, or
- Any Associates degree and 1 year of highly relevant paid work experience, or
- A combination of 1 year of highly relevant paid work experience and formal training in clinical and case management skills to demonstrate an ability to successfully perform the job.
- A relevant master's degree in social work, psychology or counseling is highly preferred.
- Bi-lingual in Spanish/English preferred.
