The Manager of the Adult Day Program is responsible for the day-to-day operations of the day center and coordination of all care delivered to participants. This includes the overall planning, development, organization and progressive direction of the operations. The Manager serves as the catalyst to the achievement of continuous quality improvements in all areas of the Program and demonstrates leadership skills in, but not limited to, the following areas: recruitment, staff development, system analysis, participant centered services, management of staff, and partnering with service providers.

Essential Functions and Responsibilities

Ensures the efficient processing and assessment of potential and existing participants.

Facilitates the development and implementation of participant care plans.

Ensures that documentation of family meetings is maintained and changes are recorded in participant care plans.

Ensures that notification is sent to appropriate agencies and regulatory bodies where indicated when care plans are modified.

Ensures the effective operations of community life, nutrition, personal care, clinical services, rehabilitation and other services of the program within the standards and regulations of the program.

Develops, recommends, and monitors the annual budget for program operations.

Prepares and submits statistical and other reports required by management, funding or contracting agencies.

Coordinates with contract services for transportation, home health, long term care, medical services, etc.

Monitors and evaluates the care planning process to assure applicability to serving the participant's needs. Identifies, develops and implements changes to the care planning system to assure efficiency, accuracy and compliance.

Ensures that staffing is maintained at the appropriate levels to provide safe and effective care. Recommends staffing levels in planning and budget development.

Coordinates orientation and inservice programs for staff to meet regulatory requirements and support performance improvement.

Makes recommendations for capital equipment. Authorizes and provides oversight of purchasing for small equipment and supplies.

Ensures the appropriate maintenance of equipment and furnishing in the program, including durable medical equipment.

Manages and evaluates the quality of service for all contracted services.

Assists in the development of agreements and contracts with provider agencies to ensure comprehensive service availability.

Coordinates with marketing to establish and maintain census goals.

Monitors the compliance of the clinical program with federal, state and local laws and regulations, reporting and correcting deficient areas.

Assures compliance of all contract personnel with established federal, state and local laws and regulations.

Assesses the need for, develops and implements educational programs for staff, participants and community.

Ensures the development and implementation of programs and services to meet the needs of participants who present special needs.

Reviews and acts on complaints filed by staff, participants and families.

Leads all regulatory surveys.

Completes appropriate reports to the Associate Director as required.

Performs other duties as assigned.

Bachelors' degree preferred.

LPN required

Must be proficient in Microsoft Office, Outlook and other computer applications.

Four (4) or more years of health care experience as a manager in a long term care facility, managed care, community based or outpatient health care setting.

Must understand clinical operations in a capitated reimbursement environment.

Must have one year experience with the frail or elderly population. Dementia experience preferred.

Supervisory experience preferred.
