Age ( adult, geriatric) and disability appropriate evaluation results establish a baseline and plan of treatment that facilitates improvement in patient function.

Evaluates patients' abilities and limitations
Establishes treatment goals based on the evaluation and reason for the referral of neuromuscular components of movement and stability; sensation; perception; manual muscle test; range of motion; body mechanics; perception; a vocational and prevocational abilities (adults only); developmental (Peds only) and other tests appropriate for the patient and the profession.
Evaluates home or work environments and recommends physical barrier modifications or ergonomic adaptations.


Treatment is appropriate for age ( adult or geriatric) as well as disability and is revised as necessary, resulting in optimal patient improvement.

Administers therapy according to the established plan, utilizing appropriate procedures, equipment and techniques within the scope of practice of Physical Therapy.
Recommends equipment, completes the equipment justification forms and arranges for the delivery of the equipment.
Re-evaluates patients, assesses their progress and makes recommendations regarding discharge (if applicable)
Evaluates home or work environments and makes physical barrier modification recommendations or ergonomic recommendations.
Consults and co-treats with therapists from other disciplines
Facilitates orthotic/prosthetics.


Documentation complies with professional, agency and payor requirements and is usable by others.

Consistently produces legible, accurate, timely concise documentation: medical records, letters to physicians.
Information is easily understood and pertinent to the patient's plan of care, reflecting patient condition, response to treatment and functional outcomes.


Written and verbal communications are tailord to meet age/educational levels of patients families and other customers and are delivered and received in a timely, clear and appropriate manner.

Education is provided for the neonate, pediatric, adolescent, adult and geriatric patients that facilitates recovery, self-care and health maintenance, health promotion and wellness.

Educates the patient and the caregiver about the goals of the physical therapy program, methods of accomplishing goals, how to perform the home program and injury prevention.


Participation and support is provided to achieve program, team and department goals.

Participates in continuous quality improvement projects
Supervises the patient care activities of aides, assistants, attendants and volunteers
Assists with departmental maintenance and supplies


Professional knowledge is advanced to ensure quality, age and disability appropriate patient care and program excellence
Participates in in-services, medical conferences, staff meetings, continuing education courses, team meetings and/ or other related activities as appropriate
Provides in-services for staff, other departments and the community, as appropriate



Bachelor's degree from an accredited Physical Therapy program approved by the American Physical Therapy Association.


Able to render patient services and may supervise Licensed Physical Therapy Assistants (LPTAs), Therapy Aides and students.

Excellent organizational skills to facilitate program development.

Demonstrates competency, as appropriate to the ages of patients served, in the ability to obtain and interpret information in terms of the patients needs; knowledge of normal growth and development; and an understanding of the range of the treatment needed.

Skilled in home program development and family training

Knowledge in appropriate selection and use of DME (wheelchairs, walkers, etc.)

Computer skills.
