Job Description

Community Medical Center is seeking an experienced Palliative Care Nurse Practitioner to join their team in Missoula, Montana.  Provides palliative care for identified Community Medical Center patients in the hospital, post-acute care facilities and the patient's home. Consults and assists the referring provider, the acute care team and other members of the interdisciplinary team to coordinate the patient's plan of care. Provides consultation in palliative care, symptom management and supportive care to meet the needs of the patient as requested by the referring provider.

  • Monday – Friday schedule; 8-5
  • Must be able to coordinate and communicate effectively with other members of the interdisciplinary team
  • Establish relationships with the exiting hospital-based palliative care team members, medical directors, hospital physicians, nurse practitioners, community providers associated with Community Medical Center, nurses and other team members.  
  • Proactively identifies opportunities to improve the patient and family experience of care.
  • Participate in daily interdisciplinary medical rounds to help identify patients who would be appropriate for a palliative care consult
  • Be available on an as needed basis to support other team members with clinical advice and assist with communication with patients and families. 
  • Prepare and maintain accurate patient records and documents to support sound medical practice and reimbursement for services provided, and support of appropriate medication uses.
  • Assist in development of clinical practice guidelines/standards in support of quality palliative care. 
  • Work collaboratively with patients and their families/caregiver, providers and other staff to
  • Take appropriate and timely measures to meet the needs of the patient, their family, and the care setting staff.
  • Assist in problem solving strategies with the patient, family, PCP and setting staff to facilitate safe care/discharge of the patient
  • Work with Administration to develop and implement the growth of the current palliative care program based on the Clinical Guideline for Excellence in Palliative Care. Edition no. 4. 

Minimum Education & Experience Requirements

  • Graduate of an accredited Geriatric, Adult or Family Nurse Practitioner Master's/Doctorate program.
  • Board Certified APN (as a Geriatric, Adult or Family Practitioner preferred)
  • Three years of experience as a nurse practitioner in clinical nursing of geriatrics, adult/family practice preferred.
  • Advanced certification in hospice and palliative nursing preferred: required within 18 months of hire
  • Experience with home health, hospice, and palliative care strongly preferred.

Professional Requirements: 

  • Maintain appropriate licensure and certifications, including DEA registration
  • Practice within established protocols for prescriptive authority, including prescribing of scheduled drugs.
  • Knowledge of home/hospice regulations, clinical practice, end of life care services, and advanced care planning, including Providers Orders for Life-Sustaining Treatments (POLST).
  •  Demonstrate excellent teaching skills in relying medical information to the patient, family, and nursing staff 
  • Possess excellent communication, interviewing, and counseling skills, and the ability to explain medical problems and treatments in accurate and understandable lay terms.
  • Must be able to coordinate and communicate effectively with colleagues, managers, and medical staff and be able to teach and develop others.

Working Conditions:

  • Clinical hours are Monday through Friday 0800-1700. Designated holidays are off.
  • Works in community care setting (hospital, post-acute care, ALF's and home ).

About Community Medical Center

Community Medical Center is located on a 45-acre campus in Missoula, Montana, and has been providing obstetrical and newborn care longer than any other facility in Missoula County. In addition, it is one of only three facilities in the state offering high-risk obstetrical care and neonatal intensive care. A neonatal referral center, we receive mothers and infants from throughout western Montana and Idaho. Maternal and neonatal emergency transport services are available.
