**Post Acute Care Opening in Las Vegas, NV**
Seeking a physician who is board certified in internal medicine or family medicine with a passion to serve older adults, who enjoys working with patients in all levels of post-acute care including LTAC, sub-acute rehab, and inpatient rehab.
* M-F, some weekends
* Work alongside pharmacists on medication management
* Complete Medicare required visits
* Complete acute, follow up, and discharge visits as necessary
* Implement an individualized plan of care for patients' goals
* In collaboration with the IDT determine transitional care
* Make recommendations on DME, home health services, or outpatient services after short stay
