This opportunity is for a part-time (with benefits; 3.5 days per week) post-acute care medical director to join our well established Connecticut/Rhode Island (Mystic, CT/Westerly, RI) post-acute team. In this role you will work in a skilled nursing environment providing primary care services. This exciting opportunity offers professional autonomy, a flexible schedule and uncapped earnings.
Position Overview:
- Provide primary care services in post-acute care facilities including skilled nursing and assisted living to a patient population of long-term care, short-term rehab, memory care, and clinically complex patients
- Drive high quality, safe and cost effective care of patients in our local post-acute care communities
- H&P, admission, progress, transfer, discharge, and acute/follow up
- Work closely with hospice, palliative care, and home care agencies to ensure a safe transition home
- Document via our EMR (Gehrimed) in a timely fashion
- Provide oversight, guidance, and mentorship to our clinicians within your facilities
- Conduct clinical case reviews as needed and share tools, information, and guidelines as they relate to quality of care
- Collaborate with leadership to review care coordination initiatives and develop collaborative intervention plans
- Background in primary care, internal medicine (IM), family medicine (FM), emergency medicine (EM), urgent care, physiatry, PM&R or geriatrics preferred
- Monday through Friday schedule with flexible daily hours
- No overnight call; 7pm to 7am call coverage is facilitated by our APC call team
- Compensation includes a base salary, productivity increase potential and directorship stipend
