Compensation and benefits: Our compensation is a productivity-based model (Fee for Service). Expected compensation is estimated income range of $133,624 to $192,417 annually with no cap on productivity bonus income potential. This full-time role is benefit eligible.

Job Details:
- Working in the town of Westerly, Rhode Island
- 15 -20 patients per day
- This is a full-time position

Responsibilities:
- Drive high-quality, safe, and cost-effective patient care in post-acute care communities
- Collaborate with other clinician interdisciplinary teams and follow CMS guidelines
- Work closely with hospice, palliative care, and home care agencies for safe patient transitions
- Ensure timely documentation and billing through our EMR (Gehrimed) meeting Medicare guidelines
Qualifications:
- Board Certified FNP, AGACNP, ACNP, or AGPCNP
- Willing to train experienced candidates as well
- Licensed in State
