Role Overview
  • Schedule: 3-4 days a week within Monday to Friday 
  • Provide patient care in a skilled nursing facility by conducting rounds in their short-term units
  • Conduct documentation at home with provided iPad laptop within 48 hours of seeing patients

Key Responsibilities
  • Perform assessments and therapy sessions
  • Collaborate with facility staff to enhance patient care
  • Ensure timely and accurate documentation

Requirements
  • Hold a valid full license as either
    • LPC licensed professional counselor
    • LMHC licensed mental health counselor
    • LCSW Licensed Clinical Social Worker
  • Experience in healthcare or long-term care is preferred but not required

Expected compensation is estimated salary range of $44,000 to $64,000 with no cap on productivity income potential
