UPMC Community Medicine, Inc. is seeking a part-time Internal Medicine or General Medicine Physician with experience in Palliative or Hospice Care to join UPMC Your Care located in Pittsburgh, Pennsylvania.


About the Opportunity:

- The Physician works independently and in collaboration with the Senior Medical to provide care to UPMC Health Plan members living with advanced chronic illness, or potentially life-limiting conditions.

-  The physician assists with the diagnosis, treatment, and management of acute and chronic conditions by conducting a comprehensive assessment and development of patient centered plan of care, including assessment and documentation of Hierarchical Condition Category (HCC) diagnoses as appropriate.

- The physician is an integral part of the clinical management team and works collaboratively with the interdisciplinary team to manage the clinical and medical needs of the patients.

- The physician communicates proactively with CRNPs and other care team members regarding the clinical condition of their patients.

-  The position will require travel for in-home visits. Must have a valid driver’s license in good standing and reliable transportation.

-  The successful candidate will be placed according to years and months of experience for employment purposes.


Qualifications:

-  Demonstrate the knowledge and skills necessary to provide care and/or interact appropriately with the patients served as specified above

- Demonstrate knowledge of the principles of growth and development over the life span

- Possess the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to their age-specific needs

-  Provide the care needs as described in the department policy and procedures


Special Skills and Abilities Required:

-  Ability to establish and maintain positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients/families

- Ability to work productively and effectively within a complex environment

- Handle multiple/changing priorities and specialized equipment

- Critical thinking, analytical and problem-solving abilities required as related to various aspects of patient care

- Good clinical judgment

- Critical thinking skills necessary to exercise and lead others in application of acceptable standards of practice

- Mobility and visual manual dexterity

- Physical stamina for frequent walking, standing, lifting, and positioning of patients


Job Requirements:

- 5+ years of experience required

- Professional internal medicine/general medicine experience and/or palliative/hospice experience is preferred

- Current PA state license and certification are required (DO or MD)

- National certification, eligible for licensure by applicable Pennsylvania board

- Current CPR certification is required

- Act 34 Clearance required

- Automotive Insurance

- Driver’s License
