- PM shifts weekdays after 4 pm plus flexible weekend hours -- 40 hours monthly total
- 100 - 120 patients daily -- mix of synchronous and asynchronous visits
- Virtual care setting
- 3 visits per hour with scheduled breaks
- Focus on diabetes, heart health, and weight management
- Adult patient population
- Telehealth-only position
