Cardiothoracic Surgeon – San Juan VA Medical Center

1. Company & Position Overview

RICAR Business Consulting LLC (RBC) is seeking two highly qualified Cardiothoracic Surgeons to deliver onsite surgical services at the San Juan VA Medical Center, part of the VA Caribbean Healthcare System. This role directly supports the U.S. Department of Veterans Affairs in providing essential cardiac and thoracic surgical care to veteran patients in Puerto Rico.

2. Job Description Summary

This is a critical clinical role responsible for providing comprehensive cardiothoracic surgical care, including elective and emergency procedures. The position ensures timely, high-quality treatment and supports the hospital’s cardiac surgery program through patient care, consultation, and collaborative case reviews.

3. Key Responsibilities

  • Perform advanced cardiothoracic surgical procedures including but not limited to thoracotomy, lobectomy, and VATs
  • Conduct patient evaluations in outpatient, inpatient, and emergency settings
  • Provide on-call coverage for urgent/emergency surgical interventions and Cath Lab support
  • Attend scheduled clinical meetings and case reviews (e.g., Cardiac Board, M&M conferences)
  • Ensure timely documentation of all procedures and encounters in the EHR system
  • Comply with all VA clinical standards, infection control protocols, and credentialing requirements
  • Mentor and supervise health professions trainees and participate in quality improvement activities

4. Required Experience

  • Minimum of 4 years continuous experience as a Cardiothoracic Surgeon within the last 5 years
  • Proven experience with: cardiopulmonary bypass, heater-cooler systems, intra-aortic balloon pumps, Impella devices, and EVH vein harvesting systems
  • Demonstrated success providing surgical care in high-pressure, acute care environments
  • Experience collaborating with interdisciplinary surgical and cardiology teams

5. Education & Certifications

Required:

  • Medical degree (MD or DO) from an accredited institution
  • Board Certified or Board Eligible in Cardiothoracic Surgery
  • Current, unrestricted medical license in any U.S. state or territory
  • BLS and ACLS certifications
  • Active DEA registration
  • Up-to-date immunizations including COVID-19 and influenza

Preferred:

  • Spanish fluency (written and spoken)
  • VA credentialing experience

6. Work Location & Arrangement

  • Location: San Juan VA Medical Center, 10 Casia Street, San Juan, Puerto Rico 00921
  • Arrangement: On-site only

7. Schedule & Position Type

  • Position Type: Full-Time Contract
  • Schedule:
    • On-site Mondays & Wednesdays (7:00 AM–3:30 PM)
    • On-call Mondays & Wednesdays (3:30 PM–7:00 AM)
    • 24-hour on-call coverage Tuesdays, Thursdays, Fridays, weekends, and holidays
    • Availability within 1 hour for emergency procedures
  • Anticipated Start Date: September 30, 2025

8. Compensation Range

  • Hourly Rates:
    • On-site scheduled services: $270–$285/hour
    • On-call services (evening/overnight): $125–$150/hour
    • 24-hour on-call days: $2,400–$2,800/day
  • Payment Frequency:  Monthly
