* Post acute care inpatient setting
* On-call during the day Monday through Friday
* Day call only on days covering

* Deliver comprehensive primary care
* Manage acute and chronic conditions
* Develop personalized care plans
* Integrate and work closely with the IDT
* Partner and work collaboratively with physicians

**Compensation & Benefits**

* Compensation is fee for service (FFS); expected compensation is estimated salary range of $109,936 to $131,926 for full-time and $26,139 to $31,367 for part-time annually with no cap on productivity income potential with full benefits.

* Uncapped bi-weekly bonus

**Ideal Candidates**

* New or experienced clinicians
* FNP, CNP, AGNP, APRN, or PA
