* Providing comprehensive primary care services to a manageable patient panel of 15-20 patients per day
* Collaborating with interdisciplinary teams to optimize patient outcomes
* Ensuring smooth transitions of care with hospice, palliative care, and home care agencies
* Maintaining accurate and timely documentation in our EMR system (Gehrimed)

**Qualifications**

* Graduate degree as a nurse practitioner or physician assistant
* Active and unrestricted NP or PA license in New York
