Advanced Practice Clinician in Eugene, OR

Opportunity Overview:

• Working in the town of Eugene, OR
• 15 -20 patients per day
• This is a part-time position, one day a week
• Nurse practitioners (NP) candidates and physician assistants (PA) are welcome to apply
• Compensation and Benefits: Our compensation is a productivity-based model (Fee for Service) that offers you control to maximize your income potential. Expected compensation is estimated income range of $41,000 to $49,000 annually with no cap on productivity bonus income potential. 

Responsibilities:

• Drive high-quality, safe, and cost-effective patient care in post-acute care communities
• Inpatient rounding in assisted living facility (ALF)
• Collaborate with other clinician interdisciplinary teams and follow CMS guidelines
• Work closely with hospice, palliative care, and home care agencies for safe patient transitions
• Ensure timely documentation and billing through our EMR (Gehrimed) meeting Medicare guidelines

Qualifications: 

• Board Certified FNP, AGACNP, ACNP, AGPCNP, or PA-C
• Willing to train experienced candidates as well
• Licensed in state
