Required Procedures:
Upper GI endoscopy (with biopsy, foreign body removal)
Lower GI endoscopy
Colonoscopy (with biopsy and polyp excision)

Preferred Procedures:
ERCP (diagnostic and therapeutic)
EGD with cautery
PEG placement
Sigmoidoscopy (flexible & rigid)
Small bowel and liver biopsy
EUS
Esophageal dilatation/manometry and interpretation
Management of Sengstaken/Blakemore tube

Requirements:
IL license required to bid (active license only)
Board certified or board eligible
Clean history required (no malpractice or NPDB reports)
IMLC not accepted
BLS and ACLS required
Will accept multiple providers

Notes:
Preference given to candidates who can perform ERCP
EUS is not required

Must be available for a phone interview and submit NPDB form, risk questions, and contingent worker form
