• Monday - Friday, 8 am - 5 pm schedule with hybrid work arrangement
• 3 days field visits, 2 days telehealth from home
• 12 - 15 patients per day
• Outpatient primary care clinic setting
• Medicare-based geriatric patient population
• Hybrid role combining home health visits and telehealth
• Minimum 3 years adult primary care experience required
• Must be proficient with technology and confident working independently
