**Assignment Details**
- Start Date: October 24, 2025

- Urgent Dates Needed:
- Oct 24 (5a) – Nov 3 (9a)
- Nov 10 (7a) – Nov 14 (5p)
- Nov 24 (7a) – Nov 27 (5p)
- Dec 1 (12p) – Dec 5 (5p)
- Dec 8 (7a) – Dec 19 (5p)
- Dec 22 (7a) – Dec 26 (5p)
- Coverage Type: Clinic + Call
- Schedule:
- Clinic: 7a–5p, Mon–Fri (~2–5 patients per day)
- Night Call: 5p–7a
- Weekends: occasional 24-hour call coverage
- Option to cover 24-hour call only without clinic (must be noted at submission)
- Setting: Outpatient clinic + inpatient call
- EMR: Epic
- Privileges: Required
- Estimated Hospital Credentialing: 45–60 days
- Payer Enrollment: 90–120 days

**Requirements**
- Board Certified or Eligible in Urology (within 5 years of residency)
- Active IN license or IMLC
- CDS Certification and DEA (Indiana address required)
- ACLS (required)
- Robotics training (highly preferred; must be noted at time of name clear)
- Clean background preferred
- Availability for listed urgent dates (required at submission)

**Clinical Details**
- Adults only
- 2–5 patients per day in clinic
- General urology procedures required
- Robotics experience preferred but not required

**Compensation**
- Rates: Facility is accepting bids
- Malpractice: Provided
- Travel: Reimbursed
