Ambulatory Care Clinical Pharmacy Specialist needed in the Midwest CPH# JOB-3095663

A nonprofit health system in the Midwest is seeking an Ambulatory Care Clinical Pharmacy Specialist to join its primary care clinic. In this role, you'll collaborate with providers and staff to optimize medication therapy, provide direct patient care under a Collaborative Practice Agreement, and serve as a clinical resource across multiple care sites. This is a full-time weekday position with no weekends, offering the chance to take ownership of an ambulatory pharmacy program within a supportive environment.

Candidates must be graduates of an ACPE-accredited School of Pharmacy, with a PharmD preferred. Post-degree ambulatory care pharmacist experience is required, and PGY2 Ambulatory Care residency training is highly valued. The ideal candidate is a proactive, collaborative professional who can work independently while driving pharmacy services forward.
