Job Description
-
Join a large physician-owned multispecialty group as a Nurse Practitioner
-
Provide care for homebound patients and local skilled nursing facility (SNF) patients
-
Deliver proactive, efficient, and cost-effective managed care
-
Perform house calls and facilitate transitions from hospital/SNF to home
-
Reconcile medications, educate patients/caretakers, diagnose conditions, and order testing
-
Communicate with visiting nurse agencies, PCPs, specialists; coordinate care and referrals
-
Collaborate with other NPs and facility medical directors in SNF settings
-
Part-time schedule (32 hours per week) with competitive salary and benefits (health, malpractice, 401K, CME allowance, PTO)
-
Must be licensed in MA (RI license a plus), DEA certificate, and MA controlled substance registration
-
Seeking candidates with maturity, organizational skills, independence, flexibility, and problem-solving ability
-
Portuguese fluency is a plus, but not required
-
Vibrant location with easy access to city amenities and scenic coastal areas
-
20 minutes from Providence, hour from Boston, and 30 minutes from Cape Cod
