• Status: Full Time
• Schedule: Monday – Friday, 730 AM - 4:30 PM

The Registered Nurse (RN) must have the following qualifications:
- Degree: Associates Degree of Nursing.
- Education: Must be a graduate from a college or university accredited by National League for Nursing Accrediting Commission (NLNAC), or The Commission on Collegiate Nursing Education (CCNE)
- Certification in disease-management related areas such as Chronic Care Professional (CCP) by Health Sciences Institute or Certified Health Coach (CHC) by National Society of Health Coaches; or a patient education body such as Certified Diabetes Educator (CDE) by National Certification Board for Diabetes Educators or Certified Asthma Educator (AE-C) by National Asthma Educator Certification Board is highly desirable. It is highly recommended candidates have at least one of these certifications
- Experience: Six years of broad-based clinical nursing experience with three years of progressively increasing managed care responsibilities with a focus in Disease Management is highly recommended. Experience with disease management, clinical practice guidelines, performance improvement, and personnel management is strongly preferred. Full-time employment in a nursing field for at least 12 of the last 18 months is mandatory. Possess a thorough clinical knowledge and breadth of experience to be able to contribute to population-level assessment and management, identify appropriate use of CPGs, conduct patient self-care education, and track patient outcomes. Extensive knowledge of DM components: outcomes measurement, collaborative practice, CPGs, patient self–management, population identification processes, and process improvement.
- Licensure: Current, full, active, and unrestricted license to practice as a Registered Nurse.
