Summary:

The LVN Patient Navigator serves as an advocate and patient care coordinator ensuring seamless coordination of services throughout the continuum of care. The LVN Patient Navigator serves as a point of contact for physicians, patients, caregivers, and medical institutions to provide resources and assistance with accessing surgical, preventive, diagnostic, therapeutic services, as well as clinical trials offered within CHRISTUS Health and in the community. At all times the LVN Patient Navigator will maintain patient confidentiality and comply with HIPAA regulations.

Responsibilities:

  • Maintains patient confidentiality and complies with HIPAA standards at all times.
  • Guides patients to resources for understanding their diagnosis and treatment options including appropriate clinical research studies and technologies for eligible patients.
  • The joint replacement navigator is an advocate, educator, and experience guide for the surgical patient as well as family members, through the total joint replacement continuum
  • Pre-op prep to include coordination with primary surgeon and consult providers to assure all pre-surgical work up and treatments are done
  • Patient and family class on expectations before, during and after surgery- medications, pain management, physical therapy, return to home
  • Follow-up patient/family phone calls, post op needs and patient experience
  • Collection and analysis of data, metrics, and other information to populate and manage certification expectations Ongoing nursing education/reinforcement of patient care guidelines

Requirements:

Education/Skills

  • Certificate Program required

Experience

  • 2 years of clinical experience required

Licenses, Registrations, or Certifications

  • LVN required
  • BLS required
