**Rates**
- Regular: $350
- Orientation: $350
- Guaranteed Hours: 8
- Overtime: $525
- Holiday: $525
- Nightly Call - Shift Rate: $450
- 24 Hour Call - Shift Rate: $2300.00
- 24 Hour Call Hours (Gratis): 3
- Call-Back: $450

**Submissions Requirements**
- Active WA license or IMLC
- Board Certified
- ACLS and BLS
- Clean malpractice and background

**Skill Requirements**
- Upper GI endoscopy (biopsy, foreign body removal, fulguration of bleeder, diagnostic ERCP)
- Esophageal dilatation and manometry (including interpretation)
- Small bowel biopsy
- Lower GI endoscopy
- Sigmoidoscopy (flexible and rigid)
- Colonoscopy (biopsy and excision of polyp)
