Seeking a full-time (4 days per week) primary care medical director role providing patient care in a post-acute care setting in a skilled nursing facility located in the Oxford, NY.

* Primary care rounding in skilled nursing facilities
* Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
* Collaborate with clinicians, facility leaders and others regarding care and services
* Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery
* Provide oversight, guidance, and mentorship to our nurse practitioners
* Conduct clinical case reviews as needed and share tools, information, and guidelines as they relate to quality of care
* Collaborate with leadership to review care coordination initiatives, develop collaborative intervention plans, and share ideas about network management issues

**Compensation & Benefits**

* Expected compensation is $231,800 to $246,651 annually with no cap on productivity income potential.

**Qualifications**

* Minimum of 1 year primary care experience
* Post-acute care experience a plus
* Active and unrestricted New York license or application in process
