Job Description
We are seeking a Family Medicine Physician to join our innovative care team in West Point, GA. Deliver whole-person care in 30- to 60-minute appointments, supported by a health coach and care team, focusing on high engagement, managed care, and culture transformation to reduce costly service utilization and improve overall health outcomes.
Position Highlights: See 10-15 patients daily in longer, meaningful appointments. Collaborative, team-based approach with health coaching support. Focus on proactive, value-based care
Requirements:
- MD or DO degree with completed Family Medicine
- Board Certified
- At least 3 years of primary care experience
- Current, valid medical license and DEA number
- Proficiency with EMR systems (NextGen preferred), coding, and Medicare Advantage/HCC documentation
- Experience with Google Workspace and team-based care
West Point, Georgia is a small, welcoming town located in Troup County on the western edge of Georgia, right along the Chattahoochee River and the Alabama state line. It’s part of the LaGrange metropolitan area and sits conveniently between Atlanta, GA and Montgomery, AL, making it accessible yet quiet.
