Position Overview:
A full-time Nurse Practitioner (FNP/ANP) or Physician Assistant (PA-C) is needed to join an Internal Medicine practice in Salida, Colorado. This position focuses on providing comprehensive, outpatient adult primary care with an emphasis on chronic disease management, preventive services, and timely access for acute needs. Both experienced providers and well-prepared new graduates are encouraged to apply.

Key Responsibilities:
• Provide comprehensive adult primary care, including histories and physicals, chronic disease management (e.g., diabetes, hypertension, COPD, CKD), preventive health, and acute visits.
• Coordinate transitions of care, including hospital and emergency department follow-ups.
• Perform common in-clinic procedures within scope of training (e.g., joint injections, minor dermatologic procedures, EKG interpretation).
• Maintain accurate and timely documentation while practicing evidence-based, patient-centered care.
• Participate in quality improvement initiatives and team-based meetings to enhance clinical operations.
Qualifications:
• Master’s degree in Nursing (FNP or Adult/Gero NP) or graduation from an accredited Physician Assistant program.
• Eligible for Colorado licensure and national certification (AANP, ANCC, or NCCPA).
• Prescriptive authority and DEA eligibility; BLS required (ACLS preferred).
• Internal Medicine or Primary Care experience preferred; strong new graduates welcome to apply.
• Excellent clinical reasoning, communication, and patient education skills; ability to work both independently and collaboratively.
