**APP Internal Medicine Opening in Muskegon, MI**

Full-time (4-5 days/week) or Part-time (3 days/week)
We are seeking a full-time or part-time NP or PA with an interest in Internal and Adult Medicine. In this role, you will be responsible for driving high quality, safe and cost-effective care of patients in our local post-acute care communities.
* Full-time role is 4 days per week with day call weekend rotation.
* You will work with an interdisciplinary team rounding on all patients as medically necessary as required by CMS.
* This includes but is not limited to H&P, admission, progress, transfer, discharge, and acute/follow up.
* You will also work with the facility to transfer short term rehab patients to the level of care most appropriate for their medical needs; including returning to home, independent living centers, assisted living, and/or memory care.
* Our NP/PAs work closely with hospice, palliative care, and home care agency to ensure a safe transition home.
* Being action oriented and having a "can do" attitude with a high standard of quality and safety in care is a top priority for our teams to help build a strong reputation in our community to support our patients.
**Compensation and Benefits**
* Compensation is fee for service - Expected compensation is estimated salary range of $100,020 to $118,025 annually with no cap on productivity income potential.
