Family Medicine / Internal Medicine Physician – Outpatient Primary Care (Hobart, IN)

Location: Hobart, IN (Outpatient)
Schedule: Full-time, Monday–Friday (standard clinic hours)
On-call: 1:10 rotation
Start: Upon completion of credentialing (90120)

Patient population & scope

• Adults (Family Medicine may also see pediatrics and geriatrics)
• Average daily volume: ~20–26 patients/day
• Services: preventive care, chronic disease management, acute care, consults and follow-ups

Facility & team

• EMR: Epic (HIMSS Stage VII)
• Support: APPs, nurses, multidisciplinary support staff
• Specialty backup: System physicians and subspecialty support
• Leadership: Reports to Practice Medical Director
• Reason for opening: Growth/expansion

Requirements

• Board: BC/BE Family Medicine or Internal Medicine
• Licensure: Eligible for Indiana medical license
• Certifications: DEA, CSR, BLS required
• Experience: Outpatient experience preferred but not required
• Other: Team-based care approach; EMR proficiency

Credentialing

• Timeline: Completion required before start (90120)
