Schedule: Monday Friday, standard clinic hours
On-Call Expectations: 1:10 rotation
Start Date: Upon completion of credentialing (90120 days) PATIENT POPULATION & SCOPE
Patient Demographics: Adults (Family Medicine may also see Pediatrics and Geriatrics)
Average Daily Volume / Census: 2026 patients per day
Consults & Follow-Ups: Yes
Case Mix/Clinical Focus Areas: Preventive care, chronic disease management, general internal medicine, and family practice FACILITY & TEAM DETAILS
EMR System: Epic fully integrated HIMSS Stage VII
Specialty Backup: Collegial system physicians and subspecialty support
Support Staff: Multidisciplinary team with APPs, nurses, and support staff
Leadership & Reporting Structure: Reports to practice Medical Director

Deliver comprehensive outpatient primary care services
Provide preventive care, chronic disease management, and acute care visits
Participate in call rotation (1:10) as scheduled

Maintain accurate and timely documentation in Epic EMR COMMUNITY & LOCATION HIGHLIGHTS
