- Active CA License

- PAs must have a CA Controlled Substance Education Course Certificate
- NPs must have a Furnishing License with Schedules II
- CVs must have month & date on work experience w/ all gaps in employment accounted for
- Immunizations such as Flu Vaccine, TB Status/X-Ray Results etc
