Job Details
- 15 patients per day
- Full-time position: 4-5 days a week
- Compensation and benefits: our compensation is fee for service (FFS); expected compensation is estimated salary range of $261,748 to $310,098 annually with no cap on productivity income potential. This full-time role is benefit eligible.
Key Responsibilities
- Perform primary care in a post-acute setting for short term and long-term patients
- Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
- Order and interpret lab results
- Prescribe and monitor medications
- Consult with physicians and coordinate patient care with other healthcare practitioners
- Educate patients and their caregivers on coping skills to handle chronic and age-related conditions
- Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery
- Promote healthy lifestyle changes
Qualifications
- Board certified internal medicine (IM), family medicine (FM), or emergency medicine (EM) clinician
- Willing to train experienced candidates as well
- Licensed in state
