Remote RN Case Manager — Diabetic Care (Contract, Pittsburgh, PA)

We're looking for an experienced RN to join a remote case management team focused on diabetic care. This is a 21-week contract/travel assignment based in Pittsburgh, PA, with an ASAP start. You’ll work days, 8-hour shifts (40 hours/week).

You’ll be the primary remote contact for patients with diabetes — assessing needs, building and updating individualized care plans, coordinating with the care team, and ensuring patients get the support they need to manage their condition safely at home.

What you’ll do:

• Conduct virtual assessments and ongoing monitoring of diabetic patients
• Create, implement, and adjust individualized care plans based on patient needs
• Coordinate care and collaborate with multidisciplinary healthcare teams
• Document patient interactions and care activities according to facility standards
• Follow state regulations and organizational policies for practice
• Protect patient privacy and handle sensitive data securely

What we’re looking for:

• Active New York RN license (note: Nurse Compact / multi-state license required)
• Minimum of 3+ years’ experience caring for patients with diabetes
• Experience in care coordination or case management is preferred
• Strong communication, critical thinking, and organizational skills
• Reliable technology for telehealth work (your own computer, webcam, microphone/speakers, phone, and stable internet)
