Requirements:
- Active New York RN license
- Nurse Compact / Multi-state license (required)
- Minimum 3+ years of diabetic care experience
- Proven clinical experience in care coordination or case management (preferred)
- Must provide your own technology (computer, webcam, microphone, speakers, phone, stable internet connection)
- Strong communication, critical thinking, and organizational skills
Responsibilities:
- Provide remote case management for diabetic patients
- Conduct virtual assessments and ongoing monitoring
- Develop, implement, and modify care plans based on patient needs
- Collaborate with multidisciplinary teams to optimize patient outcomes
- Document all interactions and updates in accordance with facility standards
- Ensure compliance with state regulations, professional guidelines, and organizational policies
- Maintain confidentiality and patient data security in a remote environment
