* Full-time
* Monday - Friday
* Flexible weekday schedule
* Perform primary care in a post-acute setting for short term and long-term patients
* Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
* Order and interpret lab results
* Prescribe and monitor medications
* Consult with physicians and coordinate patient care with other healthcare practitioners
* Educate patients and their caregivers on coping skills to handle chronic and age-related conditions
* Promote healthy lifestyle changes

**Compensation & Benefits**

* Estimated compensation range of $210,529 to $271,529 annually with no cap on productivity income potential.

**Requirements**

* 1+ years primary care experience required
* Direct post-acute care experience a plus, but not required
* Active and unrestricted Pennsylvania license
