Seeking a Post Acute Care Medical Director in Albany, NY. Experience as a medical director in a skilled nursing facility preferred. However, clinicians who are board certified and experienced in the following disciplines are encouraged to apply: family (FM), primary care, internal (IM), emergency, (EM), adult, post-acute, home health, hospice, palliative, urgent care, and hospital medicine (HM).

**Responsibilities**

* Conduct clinical case reviews, share tools, information, and guidelines as they relate to quality of care
* Collaborate with leadership to review care coordination initiatives
* Develop intervention plans, and share ideas about network and risk management issues
* Work alongside pharmacists on medication management
* Implement an individualized patient care plans
* Timely response in chart completions and pharmacy recommendations
* Provide oversight and mentorship to our clinicians in the greater Albany, New York, area

**Compensation & Benefits**

* Compensation is fee for service. Expected compensation is estimated salary range of $310,000 to $345,000 with no cap on productivity income potential.
* Uncapped bi-weekly bonus
* $25,000 sign-on bonus
