We’re looking for an experienced Registered Nurse to join a home health team in St. Johnsbury, VT. This contract/travel role starts ASAP, runs Monday–Friday on day shift, and pays $2,182 for 36 hours. If you enjoy autonomous practice, meaningful one-on-one patient care, and working with families in the home setting, this could be a great fit.

What you’ll be doing

• Perform thorough assessments and create individualized care plans, then evaluate and adjust them as patients’ needs change.
• Deliver hands-on nursing care, including medication administration, wound care, and management of chronic conditions.
• Teach patients and caregivers about disease management, medications, lifestyle changes, and home-based self-care.
• Track and document patient progress, report significant changes to physicians, and coordinate with the interdisciplinary team.
• Follow infection control, safety procedures, and regulatory requirements to ensure high-quality, compliant care.
• Provide emotional support, resource guidance, and practical home-care advice to patients and families.
• Contribute to quality improvement projects and pursue ongoing professional development.

What we’re looking for

• Current RN license in the state of practice.
• 1–2+ years of nursing experience; home health experience is a plus.
• Strong clinical assessment, critical thinking, and communication skills.
• Comfort with independent practice and managing multiple patient visits efficiently.
• Valid driver’s license and reliable transportation for travel between patient homes.
• Basic Life Support (BLS) certification required; additional certifications are a plus.
