- Ability to work independently and manage a home-visit caseload.
- Reliable transportation for patient visits.
Responsibilities:
- Perform in-home patient assessments and develop individualized care plans.
- Provide skilled nursing care including wound care, medication management, and health education.
- Monitor patient progress and adjust care plans as needed.
- Coordinate services with physicians, therapists, social workers, and caregivers.
- Document assessments, interventions, and patient outcomes accurately and timely.
- Educate patients and families on disease management, safety, and home care instructions.
- Ensure compliance with regulatory, agency, and quality standards.
