Requirements:

• Active RN license in the practicing state and current BLS certification.
• Previous home health, case management, or community nursing experience preferred.
• Strong assessment skills, clinical judgment, and ability to coordinate care.
• Comfortable working independently while managing a home-visit caseload.
• Reliable transportation for patient visits.

Key responsibilities:

• Perform comprehensive in-home assessments and create individualized care plans.
• Deliver skilled nursing services, including wound care, medication management, and patient education.
• Monitor patient status, revise care plans as needed, and document assessments, interventions, and outcomes accurately and promptly.
• Coordinate care with physicians, therapists, social workers, and caregivers.
• Teach patients and families disease management, safety practices, and home care instructions.
• Maintain compliance with agency, regulatory, and quality standards.
