Registered Nurse — Inpatient Care Coordinator (Days) — Sitka, AK

Join our Sitka, AK team as a full-time inpatient care coordinator working day shifts—start ASAP. You’ll use your clinical skills and strong communication to assess patients, create and update individualized care plans, and guide patients and families through treatment and post-discharge care. You’ll work closely with multidisciplinary teams and hospital departments to coordinate smooth transitions of care, track patient progress, and make sure documentation meets regulatory and quality standards.

What we’re looking for:

• Active RN license in the practicing state
• Experience in inpatient nursing, case management, or care coordination preferred
• Strong clinical judgment, organizational ability, and clear communication
• BLS required; CCM or care coordination certifications a plus

Core responsibilities:

• Perform thorough patient assessments and develop personalized care plans
• Coordinate care across teams and departments to support continuity and safe discharge
• Monitor and reassess patient status, revising plans as needed
• Provide education and support to patients and families about treatment and follow-up care
• Maintain compliant clinical documentation and contribute to meeting quality metrics
