Requirements:
- Active LPN/LVN license in the practicing state
- Prior experience in long-term care, skilled nursing, or rehabilitation preferred
- Strong clinical assessment, medication administration, and caregiving skills
- Effective verbal and written communication
- Ability to follow care plans and collaborate with RNs, CNAs, and interdisciplinary team
Core Responsibilities:
- Assist residents with activities of daily living and monitor physical and mental status
- Administer medications and treatments per facility policy under RN or physician supervision
- Accurately document care, observations, and interventions; report changes in condition promptly
- Maintain a safe, clean, and supportive environment
- Provide emotional support and companionship to residents
