Minimum Requirements:

• Active, valid Licensed Practical Nurse (LPN) state license
• BLS certification

Core Responsibilities:

• Provide direct nursing care under RN/physician direction: medication administration, wound care, vital signs, ADL assistance.
• Perform patient assessments (physical exam, history, medication review) and contribute to individualized care plans.
• Administer and document medications and treatments per orders and facility protocols.
• Assess, clean, dress, and monitor wounds/incisions for infection or complications.
• Monitor and record vital signs and changes in patient condition; maintain accurate patient records.
• Insert, monitor, and manage IV lines and IV medication/fluid administration when within scope of practice.
• Care for urinary catheters (insertion, maintenance, monitoring) per protocol.
• Collect specimens (blood, urine, sputum) for lab testing.
• Provide patient/family education on conditions, medications, and self-care.
• Respond to emergencies and provide basic life support as needed.
• Follow infection control standards and proper use of PPE.
• Safely handle, store, and dispose of medications and medical supplies.
• Promote preventive care (vaccinations, screenings) and support health assessments.
• Advocate for patient rights and coordinate with the healthcare team (RNs, physicians, therapists, support staff).
• May supervise CNAs or unlicensed assistive personnel as assigned.
• Other duties as assigned by management.
