Rates: Travel: $1,579/35 hrs; Local: $1,558/35 hrs

Required credentials

• Active Occupational Therapy state license
• CPR and BLS (AHA or Red Cross, with AED)

Core responsibilities

• Evaluate patients’ physical, cognitive, and environmental needs; identify limitations and establish goals.
• Develop individualized treatment plans in collaboration with patients and the interdisciplinary team.
• Implement therapeutic interventions to improve activities of daily living (ADLs), functional independence, and community/work reintegration.
• Recommend, fit, and instruct patients in use of adaptive equipment and assistive devices.
• Train and practice ADLs with patients to build practical skills.
• Educate patients and families on condition management and strategies to promote independence.
• Monitor, document, and adjust treatment plans based on patient progress; maintain accurate evaluations and progress notes.
• Communicate and coordinate with physicians, therapists, nurses, and other care team members for integrated care.
• Participate in professional development to maintain current practice standards.
• Perform other duties as assigned by management.
