Rate: Travel $2,182 / 36 hrs; Local $2,149 / 36 hrs

Required credentials:

• Valid, active Occupational Therapy state license
• CPR and BLS certification (AHA or Red Cross) with AED

Core responsibilities:

• Evaluate patients’ physical and cognitive function and home/environmental barriers; establish treatment goals.
• Develop individualized treatment plans in collaboration with patients and the interdisciplinary team.
• Implement therapeutic interventions to improve daily living skills, functional independence, and safety.
• Recommend, train in, and fit adaptive equipment and assistive devices.
• Instruct and practice activities of daily living (ADLs) and instrumental ADLs with patients.
• Educate patients and families on condition management and independence strategies.
• Monitor, document, and revise treatment plans based on patient progress; maintain accurate evaluations and progress notes.
• Coordinate care with multidisciplinary team members to support discharge planning and community/work reintegration.
• Maintain professional competence through continuing education and apply current OT best practices.
• Perform other duties as assigned by management.
