Required qualifications

• Active, valid Occupational Therapy state license
• CPR and BLS certification from the American Heart Association or Red Cross (with AED)

Key responsibilities

• Assess patients’ physical and cognitive function and home/environmental factors to identify limitations and establish goals
• Develop individualized treatment plans in collaboration with patients and the interdisciplinary team
• Implement evidence-based therapeutic interventions to improve daily living skills and independence
• Recommend, train in, and fit adaptive equipment and assistive devices
• Teach and practice activities of daily living to build patient competence and confidence
• Educate patients and families on condition management and strategies to maximize independence
• Monitor, document, and revise treatment plans based on patient progress
• Maintain thorough, accurate clinical documentation including evaluations and progress notes
• Coordinate care with multidisciplinary team members to support discharge planning and community/work reintegration
• Pursue professional development to stay current with occupational therapy best practices
• Perform other duties as assigned by management
