Requirements:

• Active RN license in the practicing state
• BLS required
• Experience in OB/GYN nursing or case management preferred
• CCM or OB/GYN care management certification preferred
• Strong clinical assessment, organization, and communication skills

Responsibilities:

• Assess patient needs and develop individualized care plans for prenatal, postpartum, and gynecologic patients
• Coordinate care with OB/GYN providers, specialists, and multidisciplinary teams
• Monitor patient progress and revise care plans as indicated
• Provide patient and family education on prenatal, postpartum, and gynecologic care and available resources
• Maintain accurate documentation and ensure compliance with regulatory and quality standards
