Registered Nurse — Case Management / Primary Care (Sitka, AK)

Join a small, community-focused team in Sitka, AK, as a full-time RN working daytime shifts. We’re looking for a practical, patient-centered nurse to support primary care and care management efforts — start as soon as possible.

What you’ll do

• Meet with patients to assess needs and create individualized care plans.
• Track patient progress and coordinate follow-up care with primary care providers and specialists.
• Teach patients and families about prevention, chronic disease management, and treatment plans in clear, accessible ways.
• Maintain accurate documentation and help meet quality and regulatory standards.
• Work closely with a multidisciplinary team to improve patient outcomes and streamline care.

What we’re looking for

• Active RN license in the practicing state.
• Experience in primary care, case management, or care coordination is preferred.
• Strong clinical judgment, organizational ability, and clear communication skills.
• Current BLS required; care management certification (e.g., CCM) is a plus.
