RN Case Manager — Specialty Services (Days) | Juneau, AK — BLS, CCM Preferred

Join our team in Juneau, AK as a full-time Registered Nurse providing specialty case management. This day-shift position is available ASAP and focuses on coordinating high-quality, patient-centered care across multidisciplinary teams.

Requirements:

• Active RN license in the practicing state
• Preferred experience in case management, specialty nursing, or care coordination
• Strong clinical judgment, organizational abilities, and clear communication skills
• BLS required; CCM or specialty case management certifications preferred

Key Responsibilities:

• Conduct comprehensive assessments and develop individualized care plans for specialty services
• Coordinate care among specialists, primary care providers, and multidisciplinary team members
• Monitor patient progress, revise plans as needed, and facilitate transitions of care
• Educate patients and families on treatment plans, resources, and disease-management strategies
• Maintain accurate documentation and ensure compliance with regulatory and quality standards
