Required credentials and skills

• Active RN license in the practicing state
• BLS certification
• Experience in case management, utilization review, or clinical coordination preferred
• Strong clinical judgment, organization, documentation, and communication skills
• CCM or specialty case management certification preferred

Primary responsibilities

• Assess patient needs and develop individualized care plans
• Review treatment plans, hospital stays, and services for medical necessity and utilization
• Coordinate care with providers, interdisciplinary teams, and payers
• Educate patients and families about care plans and available resources
• Document assessments, care plans, reviews, and ensure regulatory/compliance standards are met
