Requirements:

Active RN license in the practicing state.
Experience in case management or primary care nursing preferred.
Strong clinical, communication, and organizational skills.
BLS required; case management certification (CCM) preferred.

Responsibilities:

Assess patient needs and develop individualized care plans.
Monitor patient progress and coordinate follow-ups with primary care providers.
Educate patients and families on disease management, medications, and lifestyle changes.
Document care, interventions, and outcomes accurately.
Collaborate with multidisciplinary teams to optimize patient care and resource utilization.
