Requirements

Key Responsibilities

• Assess patient needs and create individualized care plans
• Monitor patient progress and coordinate follow-up with primary care providers
• Educate patients and families on disease management, medications, and lifestyle changes
• Accurately document care, interventions, and outcomes
• Collaborate with multidisciplinary teams to optimize patient care and resource utilization
